FLORIDA DEPARTMENT OF CORRECTIONS

FDC Assault Advisories | August 23 - August 29, 2023

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty. All assaults are investigated, and disciplinary action is imposed in accordance with policy.

Jackson Correctional Institution: On August 23, Inmate Christopher Williams (K84073) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

South Florida Reception Center: On August 23, Inmate Cecil Hendricks (V82032) assaulted an officer by striking them in the chest. Staff responded appropriately, and the inmate was subdued.

Hardee Correctional Institution: On August 23, Inmate Bogacski Thomas (M62028) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Apalachee Correctional Institution: On August 23, Inmate David Mankamyer (778916) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Union Correctional Institution: On August 23, Inmate Thomas Hensley (065362) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Century Correctional Institution: On August 24, Inmate Devin Acquisto (S26030) assaulted an officer by striking them. Staff responded appropriately, and the inmate was subdued.

Central Florida Reception Center: On August 24, Inmate Carlton Brown (987864) assaulted an officer by grabbing them. Staff responded appropriately, and the inmate was subdued.

Century Correctional Institution: On August 24, Inmate Brenton Crabtree (135237) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Florida State Prison: On August 24, Inmate Joseph Dalessandro (B08864) assaulted a staff member. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Correctional Institution: On August 24, Inmate Alejandro Graciani (R80083) assaulted an officer by grabbing them. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Wakulla Correctional Institution Annex: On August 24, Inmate Thubutu Massey (I51640) assaulted an officer by striking them with a drink on the arm and torso. Staff responded appropriately, and the inmate was subdued.

Okeechobee Correctional Institution: On August 24, Inmate Joshua Herndon (T50281) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Okeechobee Correctional Institution: On August 25, Inmate Anthony Francis (T14895) assaulted an officer by throwing a bottle striking them in the legs and waist. Staff responded appropriately, and the inmate was subdued.

Mayo Correctional Institution Annex: On August 25, Inmate Zachary Witty (V45273) assaulted an officer by kicking them in the legs. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Hardee Correctional Institution: On August 25, Inmate Lamar Davis (J59410) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Okaloosa Correctional Institution: On August 25, Inmate Jerry Pharisien (Y23189) assaulted officers by scratching their legs with restraints. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Lake Correctional Institution: On August 25, Inmate Ray Long (T54649) assaulted an officer. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Lake Correctional Institution: On August 26, Inmate Ernesto Averhoff (B11253) assaulted an officer. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Hardee Correctional Institution: On August 26, Inmate Eric Brown (T76315) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Desoto Annex: On August 26, Inmate John Eldred (721548) assaulted an officer by grabbing, pushing and pulling them. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Century Correctional Institution: On August 26, Inmate Justin McDonald (L90595) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Suwannee Annex: On August 27, Inmate Eddie Jones (Q05163) assaulted an officer by grabbing their arm and forcing them to the ground. Staff responded appropriately, and the inmate was subdued.

Hardee Correctional Institution: On August 27, Inmate Giovanni Morales (T31861) assaulted an officer by grabbing their arm. Staff responded appropriately, and the inmate was subdued.

Jackson Correctional Institution: On August 27, Inmate Justin Taylor (R39365) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Jackson Correctional Institution: On August 28, Inmate Charles Miller (122580) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Northwest Florida Reception Center: On August 28, Inmate La Trevion Butler (020324) assaulted an officer by grabbing their shirt and pulling them to the ground. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Quincy Annex: On August 28, Inmate William Crumble (N04929) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Charlotte Correctional Institution: On August 29, Inmate Stepfon Smith (N21103) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Lake Correctional Institution: On August 29, Inmate Kahlil Cruder (Q01259) assaulted an officer by striking them multiple times in the face. Staff responded appropriately, and the inmate was subdued.

Hardee Correctional Institution: On August 29, Inmate Kevin Johnson (N10892) assaulted an officer by grabbing and striking them in the wrist. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Annex: On August 29, Inmate Albert Thompson (P39579) assaulted an officer by scratching them on the neck. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.
