FLORIDA DEPARTMENT OF CORRECTIONS

FDC ASSAULT ADVISORIES | October 25, 2023 – October 31, 2023

This is a report of assault advisories that occurred during the specified timeframe. An assault advisory is issued by the Department of Corrections whenever a correctional officer or probation officer is attacked in the line of duty. All assaults are investigated, and disciplinary action is imposed in accordance with policy.

Santa Rosa Correctional Institution: On October 25, Inmate Ulysses Crouse (K81748) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued.

Calhoun Correctional Institution: On October 25, Inmate Noel Brown (X61542) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Florida Women’s Reception Center: On October 25, Inmate Lorraine Afshari (Y35064) assaulted officers by striking one in the face and striking another in the upper torso. Staff responded appropriately, and the inmate was subdued.

Avon Correctional Institution: On October 26, Inmate Alphonso Martin (C07884) assaulted officers by striking one in the shoulder and striking another on the hip. Staff responded appropriately, and the inmate was subdued.

Northwest Florida Reception Center: On October 26, Inmate Larry Major (G25334) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued.

Walton Correctional Institution: On October 26, Inmate Roderick Clark (259009) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Florida Women’s Reception Center: On October 26, Inmate Jacklyn Alaimo (N81942) assaulted an officer by striking them in the face and chest. Staff responded appropriately, and the inmate was subdued.

Reception and Medical Center: On October 26, Inmate Neil Hendricks (J02778) assaulted a staff member causing a laceration to their hand. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Lowell Annex: On October 26, Inmate Diamond Clark (E81697) assaulted an officer by punching them on the side of the head. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Okeechobee Correctional Institution: On October 27, Inmate Joshua Rudnicky (P71067) assaulted an officer by grabbing them. Staff responded appropriately, and the inmate was subdued.

Liberty Correctional Institution: On October 27, Inmate Anthony Rosier (N04616) assaulted officers by elbowing one in the face and causing a laceration to another officer’s hand. Staff responded appropriately, and the inmate was subdued.

Central Florida Reception Center: On October 28, Inmate Tyrell Clark (167233) assaulted an officer by pushing their face. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Santa Rosa Correctional Institution: On October 29, Inmate Luke Petruschke (B09393) assaulted officers by grabbing one’s leg and scratching another on the elbow. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Lowell Correctional Institution: On October 29, Inmate Taylor Pritt (C50371) assaulted an officer by biting their finger. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Suwannee Correctional Institution: On October 30, Inmate Paul Morrell (R24511) assaulted an officer by striking them in the face. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Santa Rosa Correctional Institution: On October 30, Inmate Dauviel Jackson (A90909) assaulted two officers. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Hardee Correctional Institution: On October 30, Inmate Donald Grant (T50361) assaulted an officer by striking them in the face multiple times. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Charlotte Correctional Institution: On October 30, Inmate Linell Singleton (522088) assaulted an officer by tackling them and attempting to put them in a chokehold. Staff responded appropriately, and the inmate was subdued.

Santa Rosa Correctional Institution: On October 30, Inmate Donald Grant (T50361) assaulted an officer by striking them in the face multiple times. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Holmes Correctional Institution: On October 30, Inmate Carl Leggett (M42557) assaulted an officer by striking them in the face and upper torso. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Florida State Prison: On October 31, Inmate James Roberts (132873) assaulted an officer by punching them in the head. Staff responded appropriately, and the inmate was subdued.

Florida State Prison: On October 31, Inmate Shavaughn Cooper (I45435) assaulted an officer. Staff responded appropriately, and the inmate was subdued.

Okeechobee Correctional Institution: On October 31, Inmate Dwight Williford (X73927) assaulted an officer by striking them in the chest. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Lake Correctional Institution: On October 31, Inmate Travis Tondee (Q12221) assaulted a staff member by kicking a chair, striking them in the leg. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.

Hardee Correctional Institution: On October 31, Inmate Eric Brown (T76315) assaulted an officer. Staff responded appropriately, and the inmate was subdued. Medical staff noted injuries.
